ANNEX 1 - MANDATORY CONDITIONS
No supply of alcohol may be made under the premises licence -
(a) At a time when there is no designated premises supervisor in respect of the premises, or
(b) At a time when the designated premises supervisor does not hold a personal licence or his personal licence is suspended.
Every supply of alcohol under the premises licence must be made or authorised by a person who holds a personal licence.
When a film is exhibited, admission of children to that part of the premises must be restricted in accordance with any recommendation made by the British Board of Film Classification in relation to that film.
Use of door supervisors.
Each individual in the premises who carries out a security activity must be licensed by the Security Industry Authority unless the premises or part of the premises where they are present is being used wholly or mainly as a restaurant or guest house, for theatrical performances or plays, or a Gaming Licence is for the time being in force in respect of those premises.
ANNEX 2 - Conditions Imposed in Accordance with the Operating Schedule & Under Paragraph 18(4) & 18(5) Schedule 8 Licensing Act 2003
CCTV shall be installed outside the premises and shall be maintained in good efficient working order at all times.The Licence holder shall ensure that no irresponsible drinks promotions are held at the premises.All members of staff concerned with the sale of alcohol shall be trained in relation to the sale of alcohol to person under the age of 18.All gas, electric, lighting and fire alarm installations shall be tested by qualified engineers annually.Adequate arrangements shall be in place to enable the safe evacuation of any disabled persons in an emergency.Adequate fist aid equipment and materials shall be available at the premises.Access shall be provided to emergency vehicles and shall be kept clear of any obstruction at all times.All exit doors shall be clearly sign posted and checked regularly.In the absence of adequate daylight, sufficient lighting shall be provided to all public areas of the premises.Fire safety signs shall be adequately illuminated.The capacity limit of 120 persons shall not be exceeded and an official at the premises shall ensure that the capacity limit is controlled.Clear and legible notices requesting people leaving the premises to respect the needs of local residents shall be displayed in prominent positions at all exits points at the premises.Doors and windows shall be kept closed during any entertainment that is taking place at the premises.Music shall not be played externally of the premises after 1930 hours.Staff at the premises shall be trained to prevent those leaving the premises from making unnecessary noise.Deliveries to the premises shall not take place before 0900 hours.The grounds at the premises shall be kept clear of any waste or litter.Children under the age of 16 who are not accompanied by an adult shall not be admitted to the premises between 2400 hours and 0200 hours.
