ANNEX 1 - MANDATORY CONDITIONS
No supply of alcohol may be made under the premises licence -
(a) At a time when there is no designated premises supervisor in respect of the premises, or
(b) At a time when the designated premises supervisor does not hold a personal licence or his personal licence is suspended.
Every supply of alcohol under the premises licence must be made or authorised by a person who holds a personal licence.
When a film is exhibited, admission of children to that part of the premises must be restricted in accordance with any recommendation made by the British Board of Film Classification in relation to that film.
ANNEX 2 - Conditions Imposed in Accordance with the Operating Schedule & Under Paragraph 18(4) & 18(5) Schedule 8 Licensing Act 2003
All staff who serve customers directly shall be trained to ensure that no person who is drunk or disorderly shall be able to purchase alcohol.
All reasonable requests from the statutory authorities shall be adhered to.
Staff shall observe the Premises Licence holder's Health & Safety policy.
A proof of age policy shall be applied to anyone who appears to be under the age of 21. Only photo card Driving Licences, Passports or PASS accredited cards shall be accepted as valid proof of ID.
Signs shall be prominently displayed in the premises stating that any person who appears to be under the age of 21 will be asked for proof of age.
Training shall be given to all staff who serve customers directly regarding the sale of alcohol to under 18's.
