ANNEX 1 - MANDATORY CONDITIONS

No supply of alcohol may be made under the premises licence -

(a) At a time when there is no designated premises supervisor in respect of the premises, or
(b) At a time when the designated premises supervisor does not hold a personal licence or his personal licence is suspended.
Every supply of alcohol under the premises licence must be made or authorised by a person who holds a personal licence.

Use of door supervisors.
Each individual in the premises who carries out a security activity must be licensed by the Security Industry Authority unless the premises or part of the premises where they are present is being used wholly or mainly as a restaurant or guest house, for theatrical performances or plays, or a Gaming Licence is for the time being in force in respect of those premises.
ANNEX 2 - Conditions consistent with the Operating Schedule

The premises will be closed at Police request and during specific times on the occasions of local football matches.
Only plastic glasses will be used when Burnley Football Team is playing a Category C match at home.
The premises will not have any irresponsible drinks promotions which may encourage binge drinking.
The premises will liaise with the Police regarding drinks promotions when it is deemed to be necessary.
Pre-opening safety checks will be carried out at the premises before each trading session.
Emergency exits and emergency escape routes will be maintained to ensure that these can be used at all times.
Annual testing and inspection of the Gas Installation at the premises will be carried out by a qualified engineer.
Annual testing and inspection of the Electrical Installation will be carried out by a qualified engineer.
Annual testing and inspection if the Fire Alarm at the premises will be carried out by a qualified engineer.
Doors and windows will be kept closed when entertainment is in progress.
Signs will be posted at the exits reminding customers to leave quietly.
The DJ/public announcement will ask people to leave quietly at the end of each evening.
Amplified music will be turned off in the beer garden/external areas of the premises at 10pm each night.
The management will ensure that the grounds of the premises will be kept tidy and without nuisance to other neighbouring businesses or residents.
All curtains in the licensed area will close at 11pm.
No person under 16 years to be allowed access to the premises after 8pm and persons who are between 16 and 18 years of age to be accompanied by a person over the age of 18 years.
Any S.34 gaming machines at the premises will be positioned within sight of the bar servery.
Checks on portable fire fighting equipment are to be carried out annually.
The capacity limit for the premises is 100 persons.
The premises will be a member and active supporter and participant in the local Pub Watch scheme.
CCTV system shall be maintained working at all times.
