ANNEX 2 - Conditions consistent with the Operating Schedule
1. Whenever the designated premises supervisor is not at the premises another responsible person will be nominated by the designated premises supervisor as being the person to manage the premises.
2. 2 door supervisors registered with the SIA will be on duty for the first 100 customers and thereafter one for every 100 or part thereof from 2200 to close of premises every Friday and Saturday, Christmas Eve, Christmas Day, Boxing Day, New Years Eve, New Years Day, and on Sunday’s prior to Bank Holiday Monday’s.
3. Door supervisors registered with SIA shall be equipped with an appropriate method of keeping an accurate record of the capacity levels.
4. Door Supervisors shall be positioned at each entrance/exit (other than fire only exits) to ensure that no customer leaves with any bottles or glasses.
5. Door supervisors will patrol the queue to the premises as necessary to ensure that customers who are drunk or who have acted in a disorderly manner, or have behaved in such a way as to cause a public nuisance or disturbance, are prevented from entering the premises.
6. The designated premises supervisor will maintain a daily record comprising of the following: - Start time and finish time of each door supervisor. The door supervisor will record their SIA badge number and will sign and print their name in a legible form at the beginning and end of each tour of duty. The record shall be kept securely on the premises for at least 6 months and shall be produced on request to the police or other authorised person.
7. The premises licence holder and/or the designated premises supervisor or a person nominated by them shall be a member of and regularly attend meetings of any pubwatch scheme for the area within which the premises are located.
8. A CCTV system which complies with Lancashire Constabulary’s minimum standard’s (attached) shall be installed and maintained.
9. An appropriate method of communication, as determined by the Police, will be used at the premises. (This method of communication may change in the future as technology advances). This at present refers to registration of the community/network radio scheme. The equipment must be kept in working order at all times. When the premises are open to the public the radio/equipment must be switched on and monitored at all times by the DPS or a responsible member of staff. All instances of crime and disorder must be reported via the radio and Police instructions must be complied with.
10. Wherever possible only glasses, which are made of ‘toughened/safety glass’, will be used on the premises.
11. Capacity levels shall not exceed 120.
12. Both DJ (via announcements) and other members of staff will ask people to leave quietly and have regard for residential neighbours at the end of the evening.
13. Signage will be prominently displayed at all exits to the premises asking people to leave quietly and to have regard for residential neighbours.
14. At all times the DPS or other responsible person nominated by him shall ensure that the level of noise emitting from the premises shall be kept to a minimum by regulating the noise from within the premises.
15. No persons under the age of 18(including members of staff) will be permitted on the premises under any circumstances when adult entertainment is taking place.
16. Any person who looks or appears to be under the age of 21 shall be asked to provide identification that they are over the age of 18. The following are the only forms of identification acceptable:

17. Proof of age checks will take place inside the premises by staff whenever they have doubt about a person’s age.
CONDITIONS RELATING TO PERFORMANCES OF EXOTIC DANCE
1. No audience participation will be permitted during performances of Exotic Dance.
2. Whilst performance of Exotic Dance is taking place, no person aged under the age of 18 will be on the licensed premises and a clear notice shall be displayed at each entrance to the premises in a prominent position so that it can be easily read by persons entering the premises on the following terms:
"No person under 18 will be admitted"
3. A door person shall be positioned at the top of the stairs at the entrance to the premises at all times when performance of Exotic Dance is taking place.
4. There shall be no physical contact between the customer and performer before, during or after the performance.
5. Dancers not performing must not be in the licensed area in a state of undress.
6. Notices outlining this shall be clearly displayed at every table, be on display at the entrance of the premises and each bar area.
7. Performers shall be aged not less than 18 years.
8. The Licensee must not permit the display outside the premises of photographs or other images which indicate and suggest that striptease or similar dancing takes place on the premises.
9. There shall be at least four door supervisors on the premises when performance of Exotic Dance takes place.
CCTV MINIMUM REQUIREMENTS
The licensed area of the premises shall be equipped with a CCTV system which should comply with the following criteria.
1. The system should be maintained in proper working order. Any faults will be rectified without delay.
2. The system should display on any recording the time and date of the recording.
3. The system should be operated during all normal permitted hours, any authorised extension to those hours, including drinking up times and for one hour after the latest permissible drinking up period.
4. Recordings should be held for one calendar month after the recording and should be made available to police for inspection on request
